Registration Process and Documents

Print off a registration pack (including GMS1 form), and bring it in with you prior to your first visit to the practice or on your first visit to the practice along with completed forms and Identification documents.

Alternatively, visit the Surgery to collect a registration pack to complete.

Identification documents required to register? Applies to anyone aged 16 and over.
1.      Proof of identity (passport, driving licence, birth certificate etc.)
2.      Proof of address (Identity card, official letter confirming address, we cannot accept a driving licence as proof of address)
3.      Proof of UK residency if applicable.

If you are registering a child under 16 you will need to bring a birth certificate. One or both of the parents named on the birth certificate must be registered at the practice.
